Joint Commissioning Strategy for Intermediate Care

The three year Joint Commissioning Strategy for Intermediate Care is being submitted to the Executive for approval. Intermediate Care is funded by Bracknell Forest Council and Bracknell and Ascot Clinical Commissioning Group through a section 75 Agreement. This strategy defines the future vision, principles and outcomes for intermediate care within the local area, analyses current service delivery and future demand, models for development and priorities for future development.
